Welcome, and thank you for choosing us to serve your needs. Washington Urological Associates is here to serve you, our patient, with personalized and knowledgeable urological medical care. We strive to provide the highest quality care with a sincere interest in your comfort and needs. This brochure will help you get to know us better. Please keep it handy for easy reference. New Patients A new patient packet will be mailed to you, along with a stamped return envelope. Please complete the forms and mail back to our office. This will help expedite your visit. On your first visit, please arrive 15 minutes before your scheduled time. We ask that you bring a photo ID, copies of your insurance cards, and current medications, both prescription and over the counter. Please bring any pertinent x-rays or records to your appointment. In the future, should you have any change in your information, please notify our staff so that we can update your record. We ask that you always bring your current medication list with you to any appointment.
Prescription Refills
Requests for prescription refills will be honored only during regular business hours. Please provide at least 24 hours notice of the need for any refill requests.

Vasectomy
Male Sterilization procedure preformed in office.
Circumcision
Adult circumcisions are performed in our office. Child circumcisions (ages 1 - 18) are preformed as an outpatient procedure at Beaufort County Medical Center.
Our Providers
Michael Crawford, MD
Dr. Michael Crawford received his Doctor of Medicine Degree at the
University of Oklahoma College of Medicine. He completed his surgical
and urologic residency at University of Oklahoma Teaching Hospital. Dr. Crawford is professionally interested in laparoscopic surgery, kidney stone management and treatment of prostate and kidney cancers.
Michael Lobos, MD
Dr. Michael Lobos attended St. Louis University in Missouri for his
Doctor of Medicine Degree. His surgical and urologic residency was
performed at Georgetown University Hospital in Washington, DC. Dr.
Lobos’ professional interests include genitourinary cancers, laser and microscopic surgery, along with kidney stone disease.
Sherri Hill-Coidan, FNP
Sherri Hill-Coidan is a graduate of Duke University with a Master’s
degree in Nursing. Mrs. Coidan is board certified as a Family Nurse Practitioner. Her professional interests lie in the management of incontinence, including pessary fittings, female and child urological
care and kidney stone management.
Billing Procedures
We participate with most insurance plans and file your medical claim for you. Depending upon your policy you may be responsible for a portion of your bill after the claim has been processed. It is ultimately the patient’s responsibility for the payment of any balance due on the account. Payment or co-payments are expected at the time of service. In general you will receive a bill for services provided in our office and for those procedures our physicians perform at the hospital.
