Diabetes Program
For newly diagnosed, poorly controlled and interested patients with diabetes

For information please call 252 975-4116

We offer a diabetes self-management education program that is interactive, collaborative, and an ongoing process involving a diabetes educator and the person with diabetes.

Diabetes education addresses the following self-care behaviors:
• Healthy eating
• Being active
• Monitoring
• Taking medications
• Problem solving
• Reducing Risk
• Communication with the health care provider

Referrals will be offered:
• Initial consultation sessions with a nurse and a registered dietitian
• Group education classes
• A Follow-up visit with the dietitian
We will provide feedback to your office after the education sessions have been completed.

Class participation is FREE to the public
